Sagewood Direct is our direct primary care: a membership-based primary care program your organization sponsors for employees (and optionally dependents). Members get unlimited in-person and virtual care, extended visits, direct texting, and proactive management of chronic and urgent issues. You pay a transparent per-member-per-month (PMPM) fee; we remove copays and barriers that delay care and inflate claims.

How Enrollment Works (Terms and Billing)

Employer agreement

  • Term: Initial 12-month agreement (auto-renews annually).

  • Exit: Either party may terminate with 30 days’ written notice.

  • Employee participation: Month-to-month; add/drop aligns with payroll eligibility changes.

  • Minimum: Minimum of 10 enrolled employees to activate employer-level advantages (Priority scheduling, volume discounts).

Billing & rosters

  • Monthly invoice = current enrolled members × PMPM rate ($80).

  • Pro-rating allowed for mid-month hires/terms.

  • Employer provides an updated eligibility roster monthly.

If I already offer insurance, why would I pay for DPC too?

We encourage all employees to maintain at least a catastrophic high-deductible insurance plan or a health cost-sharing option, since Sagewood Primary Care is not insurance. Think of insurance and care as two different things:

  • Insurance is for the big, unexpected expenses — hospitalizations, surgeries, or specialist care.

  • Direct Primary Care (DPC) through Sagewood is for your employees’ everyday healthcare — unlimited visits, quick access, and a doctor who knows them personally.

Just like you wouldn’t use car insurance to pay for gas or an oil change, your employees shouldn’t need to use health insurance for routine care. Insurance is for accidents; Sagewood is for keeping people healthy every day.

Do I still need insurance?

Yes, we recommend keeping insurance.

What services are not included?

While Sagewood Direct covers most of your everyday primary care needs, some services fall outside your membership. These may include:

  • Specialist Care & Hospital Services

    • Emergency room visits, hospital stays, and specialist consultations (cardiology, orthopedics, dermatology, etc.)

  • Procedures & Imaging

    • X-rays, MRIs, ultrasounds, and surgical procedures outside our clinic

  • Additional Lab Work

    • Membership includes standard labs (like cholesterol, blood sugar, thyroid) at wholesale prices

    • Specialty labs (hormone panels, advanced cardiac markers, genetic testing, food sensitivity, etc.) may incur additional charges at discounted rates

  • Medications

    • While we can often provide wholesale pricing for common prescriptions, medications are not fully included in your membership

  • Aesthetic & Regenerative Services

    • PRP joint injections for musculoskeletal conditions

    • PRP aesthetics (Vampire Facials, hair restoration)

    • Botox, fillers, and other Med Spa services

Will I get any unexpected bills?
